Duties and Responsibilities

• Help patients navigate healthcare and social service systems
• Manage care and care transitions for vulnerable populations, and reduce social isolation among patients
• Advocate for access to health benefits and resources for less privileged or vulnerable patients
• Participate in community outreach on general health, diet and exercise programs, and health risks for targeted groups
• Provide culturally appropriate health education on chronic disease prevention, physical activity, and nutrition
• Maintain clients’ notes, forms, and supplementary information through weekly checks and database updates
• Complete ongoing administrative tasks to ensure proper and organized record keeping
• Determine individuals’ eligibility and enroll them in health insurance plans
• Support the treatment of patients living with terminal illness to help them live healthier, longer lives

Targeted Specialized Knowledge

• Certificate of training as a Community Health Worker (CHEW)
• Valid practice license from the Community Health Practitioners Registration Board of Nigeria
• Minimum of 1 year relevant experience
• Fluency in a local language, with proficiency in written and spoken English
